Form	Purpose
Health History Form	Use this form to collect troop health information. Have the form available when girls attend meetings and activities. Update annually. Completed by: Parent or Guardian Returned to: Troop Volunteer
Paper Membership Form	Use these forms to register as a girl or adult member. Completed by: Parent, Guardian, or Self Returned to: Troop Volunteer or Girl Scouts of Ohio's Heartland
Online Membership Form	Use this form to register as a girl or adult member. Completed by: Parent, Guardian, or Self
Minors Photo Release	Grants GSOH permission to take and use photos of minors. Completed by: Parent or Guardian Returned to: Troop Volunteer or Event Coordinator
Accident/Incident Report (revised 2019)	Use this form when it is necessary to report an accident or an incident. This initiates an insurance claim if applicable. Completed by: Troop Volunteer Returned to: riskmanager@gsoh.org
Homeowner Agreement for Sanctioned Girl Scout Meetings	The agreement between GSOH and the homeowner for allowing meetings in a private residence. Completed by: Troop Volunteer and Homeowner Returned to: membership manager for approval by the council.
Residential Troop Meeting Request Form	The form the homeowner completes to be approved for a meeting in a private residence. Completed by: Homeowner Returned to: Customer Care
Yearly complete Volunteer Agreement	Completed yearly by ALL volunteers

Transportation Policy
Activity Permission Form	Provides parental permission for activities outside the meeting place and time. Completed by: Troop Volunteer Parent/guardian signs permission and Returned to: Troop Volunteer
Parade Waiver	Provides parental permission for marching in a parade. GSOH does NOT allow riding on floats of any kind. Completed by: Parent or Guardian Returned to: Troop Volunteer
Troop Travel Application – Level 3	Level 3 are overnight trips inside the state of Ohio This application is required at least 4 weeks before the trip. Completed by: Troop Volunteers Submitted to: Travel@gsoh.org (minimum of 4 weeks prior to trip)
Troop Travel Application – Level 4-5	Level 4 trip is overnight outside the state of Ohio but still in the country. Level 5 is international including cruises. Part 1: Level 4/5 Travel Application Part 1 - Due at LEAST 6 months before the trip for level 4 and 1 year for level 5. Serious Accident Procedures	Provides information for the volunteer in the event of serious accident, emergency, or fatality.
Service Unit Camporee Manual	Manual for a team of service unit volunteers to direct the process to ensure all aspects of the program, safety, finances, and scheduling are considered so the event can be successful.
Service Unit Finance Report	The report turned in annually by the service unit treasurer.
